Institutional Data and Safety Monitoring Board (IDSMB)
Request Process

• Requests should be submitted prior to grant or IRB submission. The investigator will then receive a data and safety monitoring board plan and cost analysis.
• Each requestor needs to complete an Institutional Data and Safety Monitoring Board (IDSMB) Request for Services form. Requests need to be delivered to the Office of Clinical Research, Health Sciences (Suite 401 Scaife Hall Room 425), faxed to 412-648-2741, or emailed to clinicalresearch@hs.pitt.edu
• The requestor needs to include in the request funding availability and person to contact regarding account information. If a waiver of these fees is being requested, please include a letter to the Associate Vice Chancellor for Clinical Research.
